Why Conservative Treatment Is Often Challenging After Spine Surgery

Spine surgery is usually considered when conservative treatments—such as medication, physical therapy, or manual therapy—fail to provide sufficient relief. However, many patients continue to experience discomfort even after surgery and ask, "Can I go back to conservative care now?" The answer is not always straightforward.
In fact, conservative treatments often become more limited and complicated after spinal surgery, for the following reasons:
1. Anatomical Changes After Surgery
Spine surgery often involves removing disc material or inserting hardware such as pedicle screws or cages. These procedures alter the spine’s natural biomechanics. As a result, treatments like spinal manipulation or traction, which were safe before, may now put undue stress on the operated area or implants, potentially worsening pain.

2. Scar Tissue and Fibrosis
Post-surgical healing typically leads to fibrosis (scar tissue formation) around nerves and soft tissue. This can result in nerve sensitivity or altered movement patterns. In such cases, certain conservative therapies may irritate the affected nerves, making the symptoms worse rather than better.

3. Limited Mobility in Fused Segments
If spinal fusion surgery was performed, the fused vertebrae can no longer move independently. This makes manual therapy, spinal traction, or posture correction techniques ineffective or risky in the fused area. Over time, the surrounding spinal segments may be overloaded, leading to a condition known as Adjacent Segment Disease.

4. Central Sensitization and Chronic Pain
Some patients continue to suffer from pain after surgery due to central sensitization, a condition where the nervous system becomes overly reactive. This heightened sensitivity means that even mild stimuli from conservative treatments can cause intense discomfort, requiring careful and specialized care.
