A Case of Coexisting Cervical Disc Herniation and Shoulder Dysfunction in a 60-Year-Old Woman

A 60-year-old female patient recently visited our clinic with complaints of neck pain, shoulder discomfort, arm tingling, and limited shoulder mobility that had persisted for several months.

She had been previously diagnosed with cervical disc herniation, and MRI findings revealed disc protrusion at the C5-C6 level, compressing the corresponding nerve root. Additionally, physical examination showed marked limitations in shoulder abduction and external rotation, with signs of supraspinatus muscle weakness.

Understanding the Connection

Cervical disc herniation and shoulder disorders (such as rotator cuff tendinopathy) often coexist, and their symptoms can overlap or influence each other.

  • Nerve root compression in the cervical spine can lead to neuromuscular dysfunction, contributing to shoulder weakness or altered motor control.

  • Conversely, chronic shoulder stiffness can cause compensatory tension in the neck muscles, such as the trapezius and levator scapulae, potentially worsening cervical spine loading.

Integrated Treatment Approach

At our clinic, we implemented the following conservative and integrated care plan:

  • Precise chiropractic adjustments focused on the upper cervical spine (C1-C2) to restore proper neural function and spinal alignment

  • Chiropractic care and functional exercises to improve shoulder joint mobility and muscular coordination

  • When necessary, intramuscular stimulation (IMS) and nerve-focused injection therapy were used to manage pain and promote tissue recovery

Treatment Outcome

After receiving care twice a week for three weeks, the patient reported reduced tingling in the arm and a gradual increase in shoulder range of motion. The combined treatment of the neck and shoulder proved effective in supporting her recovery.

The cervical spine and shoulder complex are anatomically and functionally interrelated. In cases like this, a comprehensive evaluation and integrated treatment plan that addresses both areas can significantly enhance outcomes and prevent recurrence.
