Numbness in the Face and Hand – Neurological Insights into Integrated Upper Cervical and TMJ Treatment

A patient recently visited Son Cheolho Rehabilitation Medicine Clinic with complaints of persistent numbness in the right side of the face and right hand lasting over a year.
Despite having undergone a brain MRI at a university hospital, no significant abnormalities were found. The patient also tried acupuncture therapy, but there was no noticeable improvement.

In such cases, it is essential to look beyond structural abnormalities in the brain and consider the overall functional balance of the nervous system.

Treatment Approach: Integrated Care for the Upper Cervical Spine and TMJ

Upon functional assessment, we observed misalignment of the upper cervical spine (C1–C2) as well as muscle tension and asymmetry around the temporomandibular joint (TMJ).
We implemented a combined approach, including:

  • Soft tissue stabilization around the jaw and upper neck

Neurological Mechanisms Involved

  • Upper Cervical Spine and Brainstem Relationship
    The C1–C2 area lies near vital structures like the medulla oblongata and spinal trigeminal nucleus, both of which are involved in processing facial sensation.

  • Trigeminal and Cervical Nerve Overlap
    The trigeminal nerve, responsible for facial sensation, extends downward to the C2–C4 spinal levels, meaning dysfunction in the upper cervical region can influence trigeminal nerve activity.

  • TMJ Imbalance and Trigeminal Sensitization
    Imbalance or tightness in the masseter and temporalis muscles surrounding the TMJ may lead to trigeminal nerve sensitization, contributing to facial numbness and autonomic imbalance.

  • Cervical Spine Tension and Brachial Plexus Irritation
    Misalignment of the upper cervical spine may affect dural tension and influence the brachial plexus, potentially causing altered sensation in the hand or arm.

Patient Outcome

Through integrated care focused on upper cervical correction and TMJ-related soft tissue stabilization,
the patient began to experience gradual relief of numbness and improved comfort in daily life.
