Why Do Tingling Symptoms Sometimes Worsen After Lumbar Disc Injection Therapy?

Lumbar disc herniation is a common condition that causes sciatica, leg numbness, and lower back pain. Injection therapy is often chosen to relieve inflammation and reduce nerve-related pain. However, some patients report increased tingling or numbness shortly after receiving injections. What might cause this phenomenon?


1. Inflammation and Pressure Changes Around the Nerve

The purpose of the injection is to reduce inflammation around the affected nerve. However, the injected fluid may temporarily increase local pressure or cause chemical irritation, especially in areas such as the epidural space or around the nerve root.
This may lead to temporary heightened nerve sensitivity, causing the patient to feel increased tingling sensations for a few days.


2. Temporary Response of an Already Compressed Nerve

In patients with nerve root compression due to a disc herniation, the nerve may already be hypersensitive. When medication is injected near the nerve, it can temporarily exaggerate the symptoms even if the injection is safely performed without directly touching the nerve.
This is usually transient and improves within a few days.


3. Sensory Changes During Nerve Recovery

As nerves begin to recover, it is common to experience increased tingling, burning, or abnormal sensations during the regeneration process. These are often signs that the nerve is "waking up" and healing.
In most cases, this process settles naturally within several days to a week.


4. Natural Progression of the Disease

Importantly, not all symptom changes after an injection are directly caused by the injection itself.
Lumbar disc herniation tends to have a waxing and waning course, meaning symptoms naturally fluctuate over time, regardless of treatment.
Therefore, worsening tingling after an injection may simply be part of the disease’s natural course.


Mild Soreness at the Injection Site Is Normal

It is also common to feel a dull ache or soreness at the injection site for one to two days after the procedure.
This is a result of minor tissue irritation and is not a cause for concern.
Applying an ice pack or taking a mild pain reliever usually provides sufficient relief.

So if tingling symptoms worsen slightly after the injection, it is highly unlikely to be due to nerve damage, and more likely due to a temporary nerve response or the condition’s natural fluctuation.


When to Seek Further Evaluation?

  • If symptoms gradually improve:
    → This is likely part of a normal healing response and no immediate concern is needed.

  • If symptoms worsen with weakness or loss of sensation:
    → Further evaluation, including MRI or neurological testing, may be necessary to rule out other causes.


Final Thoughts

Increased tingling after an injection does not always mean something went wrong.
It can be part of the natural healing process or the condition’s expected course.
