Case Report: Improvement of Thumb Numbness Accompanying Frozen Shoulder Through Upper Cervical Chiropractic Care

A recent patient at Son Cheolho Rehabilitation Medicine Clinic presented with bilateral frozen shoulder (adhesive capsulitis), accompanied by numbness in the left thumb. The shoulder pain and stiffness were affecting daily activities such as dressing and overhead movement, but the most distressing symptom was the persistent tingling and numbness in the thumb, which disrupted fine motor tasks like using a smartphone.

Clinical Assessment

Upon thorough history taking, functional examination, and postural analysis, it was noted that the numbness was not solely due to peripheral entrapment, such as carpal tunnel syndrome, but likely associated with functional disturbances in the upper cervical spine, particularly involving C1 and C2.

Additionally, C6 dermatome involvement—which innervates the thumb—was considered. Upper cervical misalignment can lead to biomechanical stress or neurophysiological imbalance that contributes to segmental sensitization of the cervical nerves.

Treatment Strategy

The patient received focused upper cervical chiropractic care, incorporating gentle and precise manual adjustments to improve the alignment and mobility of the C1–C2 segments. Treatment was tailored according to head rotation responses and prone leg check analysis, which indicated dysfunction primarily in the upper cervical region.

To address the shoulder issues, joint mobilization and soft tissue therapy were added, focusing on improving the scapulohumeral rhythm and reducing myofascial restrictions.

Clinical Outcome

After three weeks of integrative care (1–2 sessions per week), the patient reported a marked reduction in left thumb numbness along with improved range of motion in both shoulders. These improvements suggest that restoring proper upper cervical alignment may positively influence distal neural symptoms, especially when they stem from cervical origin.
