A Case of Improvement in Chronic Neck and Shoulder Pain After Upper Cervical and IMS Combination Therapy

A woman in her 40s recently visited our clinic due to a sudden increase in severe neck pain, accompanied by upper thoracic discomfort, limited range of motion, and long-standing bilateral scapular pain, especially around the infraspinatus muscles. She had previously received various treatments at multiple clinics, but her symptoms had not improved significantly.
She was referred to our clinic through an acquaintance. On initial evaluation, she had marked restriction in neck movement and considerable tension and pain in the mid-scapular region, which had affected her daily activities for years. A detailed assessment suggested dysfunction in the upper cervical spine, along with myofascial pain syndrome involving the thoracic region and scapular stabilizers.
We began treatment with a combination of upper cervical care and intramuscular stimulation (IMS). After just two sessions, the patient reported notable improvement in her symptoms. Her neck range of motion increased, and she experienced relief from the infraspinatus region pain that had been persistent for years.
This case highlights the importance of an integrated treatment approach in patients with complex musculoskeletal complaints. Addressing both upper cervical alignment and chronic myofascial involvement can contribute to meaningful improvements, even in cases that have not responded to previous therapies.
