[Case Review] A Patient Initially Diagnosed with Cervical Disc Disease – Later Found to Have Myelitis and Fully Recovered Through Timely Referral

by Son’s Rehabilitation Medicine & Chiropractic Clinic in Seoul

A few years ago, a patient in their 40s visited our clinic with persistent numbness in the arm. They had already been diagnosed with cervical disc herniation at another medical facility and had been receiving treatment, but their symptoms did not improve over time. Seeking a second opinion, they came to our clinic.

During the physical examination, we detected positive Hoffman's sign and other signs suggestive of long tract involvement, raising strong suspicion of a central nervous system disorder rather than a typical cervical disc problem.

Recognizing the urgency, we immediately arranged a referral to a university hospital for further evaluation. There, the patient was diagnosed with myelitis, not a cervical disc herniation. Fortunately, early treatment was initiated without delay, and the patient responded well during the acute phase. With continued rehabilitation and follow-up care, they made a remarkable recovery.

Several years later, the patient returned to our clinic for a routine check-up. They were completely symptom-free, and imaging studies only showed minor residual signs of the past inflammation. The patient expressed sincere gratitude, acknowledging that timely referral played a crucial role in preserving their neurological function and quality of life.

🩺 Key Clinical Points

  • Initial Diagnosis: Cervical disc herniation at a local clinic

  • Chief Complaint: Persistent arm numbness

  • Findings at Our Clinic: Positive Hoffman's sign and other long tract signs

  • Action Taken: Immediate referral to a university hospital

  • Final Diagnosis: Myelitis

  • Outcome: Full recovery; only minimal residual findings on follow-up imaging

This case highlights the importance of careful neurological examination and timely referral when symptoms do not align with a presumed diagnosis. At Son Cheolho Rehabilitation Medicine Clinic, we remain committed to thorough clinical evaluation, early detection of serious conditions, and collaborative care to ensure the best outcomes for our patients.
