Chronic Neck and Shoulder Pain with Right Arm Weakness – A 70-Year-Old Male Patient Case

by Son’s Rehabilitation Medicine & Chiropractic Clinic in Seoul

A 70-year-old male patient came to our clinic with progressive right arm weakness, severe neck pain, and right shoulder pain that had been ongoing for over a year. Despite visiting multiple hospitals and receiving various treatments, his symptoms had not improved, and his daily activities were becoming increasingly limited.

🧠 Initial Presentation and Clinical Findings

  • Noticeable weakness in the right arm

  • Persistent cervical spine pain and restricted range of motion

  • Diffuse pain around the right shoulder joint

  • Prior treatments, including physical therapy and medication, had minimal effect

🔍 Our Clinical Approach
Upon physical examination and imaging analysis, cervical radiculopathy was suspected, likely due to nerve root irritation in the cervical spine.
Additionally, myofascial tension in the right levator scapulae, upper trapezius, and infraspinatus muscles was noted.
Given the lack of improvement with conventional care, we focused on precise upper cervical spine evaluation and correction.

💡 Treatment Strategy

  • Upper cervical chiropractic care tailored to his biomechanical findings

  • Myofascial release and intramuscular stimulation (IMS) to alleviate deep muscle tension

  • Stepwise guidance in functional rehabilitation exercises to restore neuromuscular balance

🧾 Treatment Outcome
After just two treatment sessions:

  • The patient reported significant reduction in neck and shoulder pain (over 50% improvement)

  • Strength in the right arm began to improve

  • Activities of daily living became less painful and more manageable

🔚 Conclusion
Chronic arm weakness and neck/shoulder pain may indicate more than just muscular problems — they often involve neurological dysfunction originating from the cervical spine.
Accurate diagnosis and individualized treatment focused on upper cervical correction can make a significant difference, especially in long-standing cases.
