"The X-ray was fine, but it still hurts"—Finding a Hidden Fracture in an 80-year-old Patient

1. Persistent Pain Despite Previous Treatment
The patient had been suffering from severe foot pain. Before coming to us, they had already visited another clinic where X-rays were taken. They were told, "There are no fractures," and received an injection. However, the pain did not subside, and the swelling grew worse. 3. The "Hidden" Fracture Found
After re-evaluating the patient and performing a targeted X-ray study, I identified a fracture line that had been overlooked.
In patients in their 80s, bone density is often compromised. This can lead to insufficiency fractures or stress fractures that occur during normal daily walking—no dramatic fall is required. These fractures are notoriously difficult to spot in the early stages because the "crack" may be microscopic until the body begins the healing process.
4. Why an Accurate Diagnosis is the Best Medicine
Once the fracture was confirmed, I immediately explained the situation to the patient. To ensure they received the necessary stabilization and specialized care, I referred them to a dedicated Orthopedic Trauma Center.
The patient expressed immense relief finally knowing the real cause of their pain. Had they continued to receive symptomatic injections without addressing the underlying fracture, the injury could have progressed to a much more debilitating stage.
