Bone Metastasis Disguised as Back Pain: A Renal Cell Carcinoma Case Study

Doctors acquire knowledge through textbooks and research papers, but the true depth of medicine is often revealed by the patients we treat in the clinic. Knowing medical theory and sharply detecting critical issues in a real clinical setting are entirely different realms. Recently, the case of a patient in their late sixties undergoing long-term dialysis brought a renewed sense of clinical vigilance and learning.

1. Presentation and Initial Evaluation

The patient visited our clinic suffering from lower back pain, having been referred by a closely acquainted Korean medicine doctor to rule out any potential spinal fractures and evaluate their precise condition.

Upon arrival, the patient was walking with the aid of a cane. Immediate and meticulous palpation of the spine was performed to check for localized tenderness or typical signs of fracture. Fortunately, no acute fracture signs were observed during the physical examination.

2. Detailed X-ray Analysis and Incidental Findings

While it would have been easy to stop after checking for a spinal fracture, a more thorough, detailed review of the X-ray images was conducted to comprehensively investigate the root cause of the pain.

As a result, a distinct pathologic lesion was identified in the right pelvic bone rather than the spine. Taking this cue, a detailed medical history review revealed that the patient was currently battling renal cell carcinoma (kidney cancer).

  • Diagnostic Conclusion: This was a case of bone metastasis originating from kidney cancer that had spread to the pelvic bone.

3. Bone Metastasis and Major Cancers Prone to Spread

Cancers in the human body possess a strong tendency to metastasize to other organs, and the bone is one of the most favored sites for cancer cell colonization. Cancers known to frequently metastasize to the bone include prostate cancer, breast cancer, kidney cancer, thyroid cancer, and lung cancer.

In the medical field, these are often referred to as the "major five bone-metastasis cancers." Among them, prostate cancer, breast cancer, and kidney cancer exhibit a pronounced tendency to settle in bone tissue and cause severe pain or pathologic fractures. Therefore, musculoskeletal pain in patients with chronic illness or a history of cancer demands an exceptionally careful and comprehensive approach.

4. Post-Evaluation Measures and Clinical Recommendations

Areas affected by bone metastasis suffer from weakened bone integrity, carrying a very high risk of severe pathologic fractures if subjected to excessive physical stimulation or pressure.

Accordingly, the referring Korean medicine doctor was immediately contacted to share the patient's detailed condition and imaging findings, emphasizing the need for extreme caution during any future care before safely executing a re-transfer.

5. Final Clinical Reflections

Had we only checked for a spinal fracture without scrutinizing the broader structures on the X-ray or overlooking the patient's medical history, this could have turned into a dangerous oversight.
